If you are eligible, the program can help you get your Bayer prescription medicines for free. To find out if you may be eligible, please answer some simple questions. Click here to see if you may qualify.

You may be eligible for the Bayer US Patient Assistance Foundation if you:

 

  • Live in the United States or Puerto Rico


  • Meet certain income limits


  • Don’t have insurance, or if your Bayer prescription medicine is not covered

Yes. If you are eligible, the Bayer US Patient Assistance Foundation may be able to help you if you take any of the Bayer prescription medicines on this list.

You may be eligible to get help if your healthcare insurance does not cover your Bayer prescription medicine.

First, determine if you can get help by using our find help tool. 

 

Then, follow these steps:

 

  • If the tools says the program can help you, download the application


  • Complete and sign the Patient Information Section. A caregiver can also complete this portion of the form


  • Ask your doctor or healthcare professional (HCP) to complete and sign the Healthcare Professional Section


  • Mail or fax the completed application for review by the program

Applications are usually reviewed within 2 business days. The processing of your application may take longer if there is missing or incomplete information, including signatures. You or your doctor may be contacted about any missing information.

Upon request, you may be asked to submit copies of financial documentation. Please do not send original documents. These documents and forms may include:

 

  • Federal tax return from the prior year


  • Wage/tax statements


  • SSA-1099


  • 1099-NT


  • Proof of non-filing letter


  • Provider or patient attestation

Yes. Bayer US Patient Assistance Foundation can fax a pre-populated application to your physician for you.

No, there are no fees to apply for or participate in the program. In addition, if you are eligible, there are no copays for any medicines that you may receive from our program.

You will receive a letter in the mail when a decision has been made regarding your application. If approved, a representative will contact you by phone to schedule your shipment. The phone number that appears on your caller ID will be 1-866-228-7723.

Effective January 1, 2025, the financial eligibility requirement is 300% Federal Poverty Limit (FPL) for most Bayer prescription medications.
